The Covid-19 pandemic has highlighted the centrality of an effective and widespread territorial health system. The experience of Health Homes (Case della Salute), health facilities spread over the territory of many Italian regions belonging to the primary care sector, are considered, also in the National Recovery and Resilience Plan, the fundamental tools for relaunching social-health and social-assistance services. The article analyzes the most significant experiences of promoting and drafting guidelines for the design of Health Homes in Italy, and in particular in Emilia-Romagna, with reference also to the pioneering English experience. The article proposes to consolidate and strengthen the Health Homes as collective reference places for the community and as opportunities for urban regeneration, towards the idea of Community Homes proposed by the PNRR.
