The prevention of cognitive decay in the elderly population is becoming an important issue (for its social and economic impacts) in Europe as well as all around the world. This paper introduces City4Age, a European Horizon 2020 project, the mission of which is to create a robust, flexible, and extensible framework for the prevention of MCI/Frailty in elderly people - MCI standing for Mild Cognitive Impairment. The specific focus of the paper is on "intervention" as designed and enacted within the project: a set of highly personalized communication actions meant to suggest a healthier life-style to elderly people, also (but not only) based on their personal data regarding the main geriatric factors that are considered "indicators" of possible decay. The paper describes in detail the City4Age intervention system and outlines future developments.
